We will need the following information:
(If you want to remove any words or numbers please state that in the notes)
(If you want the shirt to be the colors in the picture please state that in the notes)

- Mascot
- Name under ball (Dad, Brother, Grandpa, Ect.)
- My Boys! or My Girls!
- 1st number
- 2nd number
- Name for sleeve 1
- Name for sleeve 2
- Design colors (Two colors only)

3. Only what is in the picture is included We do not send proofs unless purchased with the order. If you would like to add additional numbers, colors, words, or proofs see the below link to purchase this:
